I understand you want to bind your keys just the way you like it and I'm all for the settings page where you can change the keybinds, but for now you can use 'X' to open post in the list view. In all honesty it doesn't take much time to get used to new keybinds ;)

Chrome plugin has settings where you can select landing page after clicking the extension icon. I looked at the Safari extension's source code and I believe there is similar option as well.

Options would be appreciated, but you can always use Stylish and create a custom style.

In my opinion this is a problem of a site that publishes feeds, not The Old Reader. Post title is integral part of The Old Reader interface. If the title is omitted then proper placeholder should be used. I would say that post URL is good "plan B".

Describe what do you mean by "unnecessary"? Some people consider sidebar as unnecessary and because of that bookmarklet [1] that removes it was created. I don't see why any other element should be considered as unnecessary. I would suggest closing this suggestion (: P) for inaccuracy.

I would add that there should not only be a visual indication of the selected post, but the selected post should be rather "last clicked" than "the topmost post".

The Old Reader considers the topmost article as "selected" or "focused". Please vote up this [1] suggestion instead of creating new one :)

You can press L when the post is first from the top.

I found something that might be interesting for you. This is not exactly what you've described, but I think it will suit your needs. I also think this is not intended, but maybe it won't be removed. If you're in the list view and you click between the green unread indicator and post title, the post will be marked as read but it won't open. The space you have to click on is about 10px wide, so I understand this might not be very comfortable, but it's still something!

I can also advise you using keyboard shortcuts. You can "n" posts and "m" them as read if you think selected post should be marked as read.

Not fully duplicate but should be solved by this [1] or this [2]. Please consider voting on existing suggestions rather than creating a new one.

The Old Reader already has the exact thing you've described and it's even better than in Google Reader. Just click "Show all posts" or "Show unread only" on the rightmost part of the screen, just above the feeds. In Google Reader you had to choose from dropdown menu, here the button changes dynamically depending on the current setting.

    Workaround:
    - list view: use B instead of V/O
    - full view: left-click the title instead of middle-clicking

Although you have a point that both V and middle-click should mark article as read and because of that I will give you +1 vote.
